Job Summary

We are seeking a licensed practical nurse (LPN) to join our remote care team. In this role, you will provide ongoing support, care coordination, and patient education to chronically ill patients via phone and digital health platforms. If you have a passion for patient advocacy, chronic disease management, and telehealth, this position is perfect for you.

Responsibilities

  • Conduct monthly telephonic check-ins with patients to assess health status, medication adherence, and overall well-being
  • Review RPM data such as blood pressure and glucose levels and escalate concerns to providers
  • Provide patient education on disease management, medication, and lifestyle changes
  • Collaborate with physicians, nurses, and care teams to develop and adjust care plans
  • Document all interactions in the EHR system and ensure compliance with Medicare CCM and RPM guidelines
  • Assist with care coordination, including scheduling follow-ups and referrals as needed

Requirements

  • Active LPN license required
  • Experience in chronic disease management, telehealth, or primary care preferred
  • Strong communication and patient engagement skills
  • Tech-savvy with experience using EHR systems and remote monitoring tools
  • Ability to work independently in a remote setting with minimal supervision
  • Reliable internet and a quiet workspace
