Job Description

UnityPoint Health is seeking a dedicated and highly organized Remote Clinical Coordinator to join our dynamic team. In this vital remote role, you will be the central point of contact for a diverse caseload, meticulously managing patient records, scheduling appointments, coordinating diagnostic tests, and facilitating referrals to specialists. You will work closely with physicians, nurses, and and other allied health professionals to develop and implement individualized care plans, ensuring that all aspects of a patient’s treatment journey are aligned and executed efficiently. Beyond the logistical aspects, a significant part of your role will involve patient education and support, empowering individuals and their families with the information and resources they need to navigate their healthcare options. You will also be responsible for maintaining accurate and up-to-date documentation in our electronic health record system, adhering strictly to HIPAA regulations and organizational policies regarding patient privacy and data security. Key Responsibilities

  • Coordinate and schedule patient appointments, diagnostic tests, and specialist referrals efficiently.
  • Act as the primary point of contact for patients, families, and healthcare providers, ensuring clear and timely communication.
  • Maintain accurate and confidential patient records within the electronic health record (EHR) system, adhering to all HIPAA guidelines.
  • Facilitate communication between clinical staff, physicians, and external agencies to ensure integrated patient care plans.
  • Provide patient education and resources, helping patients and their families understand care plans and navigate healthcare services.
  • Monitor and track patient progress, identifying potential barriers to care and proactively resolving issues.

Required Skills

  • Minimum 3 years of experience in a clinical coordination, patient navigator, or similar role within a healthcare setting.

  • Proficiency with Electronic Health Record (EHR) systems (e.g., Epic, Cerner).
