  • Current California State NP license

  • Current Nurse Practitioner Furnishing Number (NPs)

  • Nurse Practitioner furnishing number of NPs (NPs)

  • CAPA Controlled Substance Certification (PAs)

  • Active DEA registration

Preferred:

  • 6 months or more of related primary care experience as an APC
