<div style="text-align: justify;">The Counselor II is a valuable member of our behavioral health Residential Support Team who provides comprehensive services and support to chronically mentally ill adults in their homes, the community, and the office for clients residing in the Clustered Apartment Program. </div>Counselor II Key Responsibilities <ul style="margin-bottom: 3pt;"><li>Provides direct services to clients, including organizing and facilitating individual and group treatment and activities, supervision of the milieu, case management, and outreach. </li><li>Conducts assessments and develops treatment plans with clients. </li><li>Provides individual and group counseling, skill building, and case management, as well as crisis intervention when needed. </li><li>Provides education and direction to clients, family members, and/or significant others.</li><li>Works with clients to secure and maintain entitlements, housing, and other needed services.</li><li>Monitors clients to ensure that they are caring adequately for themselves and for the facility and furnishings.</li><li>Demonstrates/teaches ADLs when required.</li><li>Organizes and participates in treatment-related clinical conferences and staffing on assigned clients.
