Counselor II - Residential Clustered Apartment Program

<div style="text-align: justify;">The <strong>Counselor II</strong> is a valuable member of our behavioral health <strong>Residential Support</strong> <strong>Team </strong>who provides comprehensive services and support to chronically mentally ill adults in their homes, the community, and the office for clients residing in the <strong>Clustered Apartment Program</strong>. <br><br></div><p data-pasted="true"><strong>Counselor II Key Responsibilities </strong></p><ul style="margin-bottom: 3pt;"><li>Provides direct services to clients, including organizing and facilitating individual and group treatment and activities, supervision of the milieu, case management, and outreach. </li><li>Conducts assessments and develops treatment plans with clients.  </li><li>Provides individual and group counseling, skill building, and case management, as well as crisis intervention when needed.  </li><li>Provides education and direction to clients, family members, and/or significant others.</li><li>Works with clients to secure and maintain entitlements, housing, and other needed services.</li><li>Monitors clients to ensure that they are caring adequately for themselves and for the facility and furnishings.</li><li>Demonstrates/teaches ADLs when required.</li><li>Organizes and participates in treatment-related clinical conferences and staffing on assigned clients.  Reports client progress, behavior problems, or need for medication evaluation to program supervisors/physicians/psychiatrists regularly.</li></ul><br><strong>Schedule: 37.5 hours a week</strong><ul><li>T<span style="color: inherit; font-style: normal; text-align: start; text-indent: 0px;" data-pasted="true">uesday to Saturday (</span><span style="color: inherit; font-style: normal; text-align: start; text-indent: 0px;">8:00 am-4:00 pm </span></li><li data-pasted="true"><strong>$5.00 shift differential for working hours deemed eligible for shift and weekend premium</strong></li></ul><ul><li>Bachelor’s degree in a related field, or</li><li>Bachelor’s in any field and two (2) years’ experience, or</li><li>High School diploma or GED and five (5) years’ experience</li><li>Valid driver’s license and vehicle insurance</li><li>Requires use of personal vehicle when a LCHD fleet vehicle is not available for case management activities, client appointments, crisis response, community outreach or other job-related activities.</li></ul><p style="margin: 0in 0in 8pt; text-align: justify;" data-pasted="true"><span style="color:inherit;">As the largest human service provider in Lake County, we believe that services must be available without barriers.
