Position Summary

The Hospice Admissions RN is responsible for initiating the hospice Start of Care process by conducting comprehensive patient assessments, determining eligibility, and educating patients and families about hospice philosophy, services, and expectations.

Key Responsibilities

Start of Care & Assessment

· Conduct initial nursing assessments, including complete physical examinations, medical history review, medication reconciliation, and symptom evaluation.

· Determine patient eligibility for hospice services in accordance with Medicare and regulatory guidelines.

· Develop and document the initial Plan of Care, Start of Care documentation, and required physician orders.

Patient & Family Education

· Educate patients and families regarding hospice philosophy, services, goals of care, and expectations.

· Provide instruction on symptom management, medication administration, comfort measures, and caregiver support.

· Explain hospice benefits, consent forms, and how to access 24/7 hospice support.

Collaboration & Communication

· Collaborate with referring physicians, hospice medical directors, and the interdisciplinary team to ensure appropriate and timely admissions.

· Communicate clinical findings and eligibility determinations clearly and accurately.

· Maintain professional relationships with referral sources, facilities, and community partners.

Direct Nursing Care

· Provide skilled nursing care and immediate symptom management interventions during the admission visit as needed.

· Coordinate care with social work, chaplaincy, home health aides, and durable medical equipment providers.

Documentation & Compliance

· Complete accurate, thorough, and timely clinical documentation in accordance with hospice, state, and federal regulations.

· Maintain compliance with infection control standards, patient privacy laws, and professional nursing practice requirements.

Additional Duties

· Participate in on-call rotation as assigned.

· Attend interdisciplinary team meetings and required trainings.

· Engage in ongoing professional development and continuing education.

Qualifications Required

· Current and unrestricted California Registered Nurse (RN) license.

· Current CPR certification.

· Valid California driver’s license with reliable transportation and current auto insurance.

· Ability to work independently with strong organizational, communication, and assessment skills.

Preferred

· Minimum of one year of nursing experience; hospice or home health experience preferred.

· Experience in public health, home health, or hospice nursing.

· Bachelor of Science in Nursing (BSN) preferred.

Physical Requirements

Ability to lift and transfer patients and equipment; prolonged standing, walking, bending, kneeling, and driving as required for patient visits.

Pay: $65.00 - $70.00 per hour

Expected hours: 40.0 per week
